RELEASE, WAIVER, AND INDEMNITY AGREEMENT

This form must be completed for each soccer Player (participant) and must be signed by the Player’s parent or legal guardian.
No Player will be allowed to participate in training, practice or games without this form properly executed and on file.

I, the undersigned, in consideration for my minor child’s voluntary participation in organized soccer, do hereby willfully acknowledge that my signature below attests to my understanding and agreement that:

The status of the Participant named above (the “Player”) will be kept in good standing. I and the Player will not compromise ourselves in such a way as to do harm to RC10 Professional Soccer Training, LLC (the “Club”), any league or tournament, knowing that the Player may be dismissed from participation, with possible loss of payment or dues, for violent conduct or unsportsmanlike behavior on or off the field of play. I agree to pay for any and all damages to any property or indemnities caused by me or the Player willfully, negligently, or otherwise. I recognize that soccer is a physical, contact sport that involves the risk of injury. I assume all risks and hazards associated with the Player’s participation in the sport. The Player is in proper physical condition to participate in soccer practices and games and has no illness, disease or existing injury or physical defect that would be aggravated by Player’s participation. I will inform the Club if this status changes. I further acknowledge that the risk in participation with the Club may involve loss or damage to me or my property, including the risk of death, or other unforeseen consequences, including those which may be due to the unavailability of immediate emergency medical care. I have a current medical consent form in force. The Player will wear shin guards, properly fitted and appropriate shoes, and other protective equipment (e.g., mouthpieces), as provided by soccer rules and as I deem necessary, to all Club events. The Club is not required to have personal injury insurance that covers my participation. Therefore, I have a current, active, personal injury insurance policy in force, which covers the Player’s participation. Under any condition, I am responsible for any and all medical expenses arising from the Player’s participation, both in practices and games and while travelling to and from these events. I have the right and responsibility to inspect the equipment and facilities prior to events and, if I believe that anything may be unsafe, I will advise the Club of the condition in writing and may refuse to allow the Player to participate. The Player’s participation assumes consent. I authorize the Player’s photograph, picture or likeness, and voice to appear in any documentary, promotion (including advertising), television, video, or radio coverage of the Club, without compensation. I authorize that an unaltered copy of this form may be generated and given to the officers or directors of other leagues or tournaments in order to allow the Player’s participation in their soccer programs, if the form is required and the Player is participating. I hereby release, waive liability, discharge, hold harmless, indemnify, and covenant not to sue the Club, its associated directors, administrators, officers, managers, employees, coaches, trainers, volunteers, sponsors and advertisers, and other agents, estates or executors, from any and all liability incurred in the conduct of, and the Player’s participation in, their soccer programs. This includes owners, lessors, and lessees of premises, municipalities, government agencies, successors, heirs, and assigns.

COVID-19: I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that the Club has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19. I further acknowledge that the Club cannot guarantee that I and the Player will not become infected with the Coronavirus/COVID-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Club staff, and other Club Players, participants and their families. I voluntarily seek services provided by the Club for the Player and acknowledge that I am increasing the Player’s risk to exposure to the Coronavirus/COVID-19. I acknowledge that the Player and I must comply with all set procedures to reduce the spread while participating in activities with the Club.

I also attest that the Player is/has:

⦁ not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell;

⦁ not traveled internationally within the last 14 days

⦁ not traveled to a highly impacted area within the United States of America in the last 14 days

⦁ not been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19

⦁ not been diagnosed with Coronavirus/COVID-19 and not yet cleared as non-contagious by state or local public health authorities

⦁ following all CDC recommended guidelines as much as possible and limiting exposure to the Coronavirus/COVID-19

I have completely read this document and fully understand its contents. I acknowledge that I, on behalf of myself and the Player, have given up substantial rights by accepting this document and that I do so voluntarily. My signature attests to this on behalf of myself, the Player and my executors, personal representatives, administrators, heirs, next-of-kin, successors, and assigns. Additionally, as the parent and natural guardian or legal guardian of the Player, I hereby agree to the foregoing Waiver of Liability and Release for, and on behalf of, the Player named above. I hereby bind myself, the minor, and all other assigns to the terms of the Waiver of Liability and Release. I represent and certify that I have the legal capacity and the authority to act for, and on behalf of, the minor Player in the execution of this Waiver of Liability and Release.
